Several years ago, I was hunting with a Dr. that sometimes worked the emergency room.
By law.... they have to "see" the people when they come in. But what treatment they gets depends on what is the problem. It still cost you and I a bunch of money to pay for them going to the ER.
The tricky part for the Hospital is that they must provide proper care for these "walk-ins" or be open for a law suit. The ER can't just look at the person and tell them to go somewhere else.
